Adrenocorticotropic hormone (ACTH)-secreting renal neuroendocrine tumor (NET) is extremely rare. A left renal mass was detected on ultrasound in a 26-year-old patient with ACTH-dependent Cushing syndrome. The lesion demonstrated slightly high density and moderate uptake of radiotracer on 68 Ga-DOTATATE PET/CT, suspected to be a NET. Renal vein sampling revealed excessive ACTH secretion from the left kidney.
