The frequency and factors affecting anxiety and depression in patients with rheumatoid arthritis.

类风湿性关节炎(RA)是一种慢性的炎症性疾病。焦虑和抑郁是RA患者面临的重要问题。本研究的目的是确定影响RA患者抑郁和焦虑的频率和因素。本研究纳入了182名18-85岁的RA患者。RA的诊断根据2010年ACR/EULAR RA分类标准进行。排除标准为精神病、怀孕、哺乳和恶性肿瘤。人口统计学数据、疾病持续时间、教育水平、28个关节计数下的疾病活动评分(DAS28)、健康评估问卷(HAQ)得分和医院焦虑和抑郁量表(HADS)是本分析所使用的参数。研究发现,50.3%的患者存在抑郁症状,25.3%的患者有焦虑症状。在抑郁和/或焦虑的患者中,HAQ和DAS28评分高于其他接受研究的RA患者。女性、家庭主妇和教育水平较低的人抑郁症状明显更高。蓝领工人焦虑症状发生比例明显更高。本研究发现,RA患者中抑郁和焦虑的发生率较高。这些结果与一般人群相比表明RA患者面临的真正问题。这指向了炎症和抑郁、焦虑之间的关系。在RA患者的身体检查中,应同时进行精神健康评估和心理状况评估。版权所有:2023年华沙国家老年、风湿性疾病和康复研究所。
Rheumatoid arthritis (RA) is a chronic inflammatory disease. Anxiety and depression are important problems in patients with RA. The aim of this study was to determine the frequency and the factors affecting depression and anxiety in patients with RA.One hundred and eighty-two patients with RA, aged 18-85 years, were included in this study. The diagnosis of RA was established according to ACR/EULAR RA classification criteria from 2010. Psychosis, pregnancy, breastfeeding and malignancy were exclusion criteria. The demographic data as well as disease duration, educational status, Disease Activity Score with 28-joint counts (DAS28), Health Assessment Questionnaire (HAQ) score and the Hospital Anxiety and Depression Scale (HADS) were the parameters used in the analysis.Depression symptoms were present in 50.3%, anxiety in 25.3% of the studied patients. In patients with depression and/or anxiety HAQ and DAS28 scores were higher than other studied RA patients. Depression was determined at significantly higher rates in females, housewives and those with a low education level. Anxiety was determined significantly more often in blue-collar workers.In the present study, depression and anxiety were observed at high rates in patients with RA. These results confirm the real problem in RA patients in comparison to the general population. This points to the relationship between inflammation and depression and anxiety. Psychiatric evaluations and mental status assessment should not be forgotten together with physical examinations of RA patients.Copyright: © 2023 Narodowy Instytut Geriatrii, Reumatologii i Rehabilitacji w Warszawie.
